Chronic obstructive pulmonary disease is a group of progressive lung conditions, principally emphysema and chronic bronchitis, that restrict airflow and make breathing increasingly difficult. Smoking is the main risk factor, but occupational exposure to dust and fumes, air pollution and other factors can also contribute. [1]

COPD was responsible for an estimated 3.4 million deaths worldwide in 2023, making it the world’s third leading cause of death. Tobacco smoking accounts for more than 70% of COPD cases in high-income countries. [2]

COPD can develop gradually, and people may attribute breathlessness, coughing or reduced exercise capacity to ageing or smoking. In the UK, NICE recommends spirometry to support diagnosis, while NHS guidance advises people with persistent symptoms, particularly those over 35 who smoke or have smoked, to seek assessment. [3]

There is currently no cure for COPD, but treatment can slow progression, reduce symptoms and help prevent exacerbations. Alongside inhaled medicines and smoking cessation, pulmonary rehabilitation combines individually tailored exercise with education and lifestyle support. In England, pulmonary rehabilitation is a key intervention within the NHS respiratory disease strategy. [4]

Management is increasingly being tailored according to symptoms, exacerbation risk and individual disease characteristics. NHS England’s 2026 COPD strategy includes assessment of biomarkers and consideration of biologic therapies for patients at rising risk, alongside infection markers, specialist review and other interventions. This reflects a broader shift from treating COPD as a single disease towards identifying different patient groups and targeting treatment accordingly. [5]
