Pharmaceutical firm Lilly’s orforglipron was associated with greater reductions in body weight and blood glucose than oral semaglutide in adults with type 2 diabetes (T2D) who were overweight or had obesity, according to an indirect treatment comparison presented at the European Association for the Study of Diabetes (EASD) Annual Meeting 2026.
The analysis compared data from the phase 3 ACHIEVE-3 and PIONEER-PLUS trials, evaluating orforglipron 17.2mg against oral semaglutide 25mg over 52 weeks.
Orforglipron was associated with a 1.46 percentage point greater reduction in body weight and a 0.31 percentage point greater reduction in glycated haemoglobin (HbA1c), a measure of average blood glucose. Both differences were statistically significant, with findings consistent across multiple sensitivity analyses.
Both treatments are oral glucagon-like peptide-1 (GLP-1) receptor agonists, but orforglipron is a small-molecule, non-peptide therapy that can be taken without food or water restrictions. Oral semaglutide has specific administration requirements, making dosing flexibility a potential differentiator.
Maria Alexandrovich, Pharma Analyst at GlobalData, said the findings could strengthen Lilly’s position in the oral GLP-1 market, while cautioning that the indirect comparison did not establish the clinical significance of the differences.
“Direct head-to-head studies and longer-term outcomes are needed to establish the clinical significance of these differences,” she said.
“Overall, the findings support orforglipron as a competitive oral GLP-1 option in T2D, combining strong glycemic control and weight-loss efficacy with simpler daily administration. Longer-term evidence will help determine whether these advantages translate into improved outcomes in clinical practice.”
